Action Group B3 - Replicating and tutoring integrated care for chronic diseases

EIP on AHA’s Action groups were (2012-2020) a community of partners committed to work on specific issues related to active and healthy ageing. They did this by sharing their knowledge and expertise with their peers, increasing the added-value of their national and local experience, and identifying gaps that need to be fulfilled at European level. Currently, Futurium is the platform of choice for partners to continue to share their work.

The Action Group on integrated care was a unique collaborative network of partners representing around 260 multi-stakeholder commitments from national and regional administrations, local authorities, research centres, academia, industry (including SMEs) and advocacy organisations from across the EU.

The partners aimed to reduce the avoidable/unnecessary hospitalisation of older people with chronic conditions. They worked towards integrated care services that are more closely oriented to the needs of patients /users, and are multidisciplinary, well-co-ordinated, accessible, and anchored in community and home care settings. Such models improved coordination between the different levels of health services and align them with social care throughout the entire health promotion and care chain. They harmonised and coordinated the management, organisation and delivery of services to make sure they provided quality and efficient solutions that respond to the needs of the patients.
