The InAdvance project proposes a new model of palliative care based on early detection and personalised care pathways addressed specifically to older people with complex chronic conditions.

The final conference will provide the opportunity to exchange with InAdvance project partners on palliative care and learn more about the findings of the project:

• Interventions supporting earlier detection of palliative care
• Improving palliative care through better communication with patients and families
• Impact and feasibility assessment of the InAdvance interventions
• Cost effectiveness of early detection of Palliative Care
